Good that you found pics and info on the net too. Yes there is a temple with Maa Chinnamasta as main deity in a village in North Bengal (I heard this too). Bengal is the state of Kali worship. Other Devi temples are scattered all over India, like Assam, Kerala, etc. when it comes to the 10 MahaVidyas. When I visited India a many, many years ago and traveled/lived from North to South, I still couldn't find a temple that had Maa Bagalamukhi as main deity.
