In any case, whatever be the matter, please do give all of the following:

 

1. Date and exact time of birth

2. Time as per hospital records

3. Details of what the nurse was doing when the time was recorded

4. How accurate was the clock

5. After how many minutes was the time actually recorded

6. Any other details that can help you slip out of a poor reading
