Would it cause any identity issues, perhaps an inability to consolidate one's real nature and persona? How would it differ for Lagna placed in bhadakasthana from the AL? I don't know if it's significant, but these situations cannot arise if Lagna is a fixed sign (first case) or the AL is a fixed sign (second case). Maybe fixed Lagna people are not the kind who have identity problems - everything's more clear-cut? AL is how people perceive us.If the AL is in Bhadhak sthan perhaps such a person will have come issues regarding what people think of them, they might be misunderstood etc.Aspects on this will change this.On the other hand if we feel things are not clear cut and are confused about our own identity then would'nt our mind need to be involved too? Perhaps afflictions to the Moon can indicate this? Just thinking with you...waiting for the Gurus to give principles on this. I suppose in that case it wouldn't be so much about identity problems, and what the native thinks. But I think it must result in some sort of obstruction relating to the manner in which the native is perceived by the world. It may make it difficult to make oneself understood. Or it may be that people don't want to listen... So nothing to do with the state of mind of the person, but rather with the situations they might have to deal with.
