This is known as Phimosis, where you cannot retract the foreskin. There is also Paraphimosis, where the skin does retract, but due to fibrosis, does not get back to the original position. In Phimosis, there is pain at erection, and inability to perform coitus, but no urinary retention, where as in the latter, there may be additional urinary retention, and obstruction. It may be congenital, or due to inadequate hygiene ( you should clean the male organ --prepuce daily), or due to diabetes ( where the residual urine creates fibrosis), or urinary tract infection from syphilis, gonorrhea, or any other sexually transmitted disease, or other causes of urinary tract infection. The answer to both Phimosis and paraphimosis is surgical correction, in addition to better daily hygiene and treatment of any underlying diabetes or any urinary infection.
